Course content
Lectures: 1. - 2. Community care - history of community care in the Czech Republic, definition, aim, structure within public health care system and related terminology. 3. - 4. Healthy community - factors influencing community health, research in community care, WHO recommendations. 5. - 6. Characteristics of complex home care and its basic principles. Anamnestics and diagnostics in home care in paediatric nursing. 7. - 8. Selected models of health support and disease prevention (Nola Pender model, Kathryn Barnard model) 9. - 10. Community care for selected target groups (children, families and groups in the community). Seminars: 1. - 2. Nursing process in community care - planning of educational unit. 3. - 4. Visiting a patient in home care (planning, preparation, nursing briefcase); nurse´s tasks. 5. - 6. Discharge of a client from home care. Record of home visits. Home care agency documentation. Experience of nurses working in AHC. 7. - 8. Application of selected models of health support and disease prevention (Nola Pender model, Kathryn Barnard model) 9. - 10. Community care for family - woman, child, man 11. - 12. Community care for school children. 13. - 14. Community care for mentally ill and mentally handicapped 15. - 16. Community care for old and dying clients 17. - 20. Anamnestics and diagnostics in home care. Home health care plans in relation to the most common diagnostic groups in paediatric patients.

Learning outcomes
The course is designed as a theoretical-practical unit. It informs about health services and institutions in community and home care, and explains primary health care of general practitioners for children and adolescents. It focuses on the health of children, families and groups in the community. The meaning of the term "community" is explained, as well as its main social function in meeting needs and its hierarchy: individual - family - municipality - city - region - state - nation - world. The students will realize the need for a healthy social and natural environment for the lives of children and their families. They will learn to treat paediatric patients in their own social environment. Goals of the course: " To understand the meaning of 'community', its main social function in meeting people's needs and its hierarchy: individual - family - municipality - city - region - state - nation - world. " To understand the need for a healthy social and natural environment in the community for the lives of individuals and their families. " To teach the treatment of paediatric clients / patients in their home environment. " To acquaint students with the organizational structure of services of home care agencies and applicable legislation for their activities.
" The student applies his or her knowledge of prevention and screening activities in conditions of complex home care. " The student applies his or her knowledge of selected models of health support and disease prevention. " The student practically applies his or her nursing knowledge about forms, structure and management of community and home care in cooperation with field nurses. " The student knows the competences and activities of nurses working in community care. " The student can practically distinguish the roles of the individual members of a multidisciplinary team and can perform them in mutual cooperation. " The student knows the principles of education and implements them in the care of children clients in community care in all health areas. " The student can identify the needs of clients of selected target groups (children, families and groups in the community). " The student implements preventive programmes in connection with the care of children and adolescents. " The student knows the principles of education about hospice and palliative care. " The student can use assessment techniques in home and community care.
